The core difference: what happens overnight

Both options mean someone is always at the home. The difference is the night:

  • 24-hour care — caregivers work in shifts (often two or three caregivers across the day) and stay awake around the clock, including overnight. No one is ever asleep on duty.
  • Live-in care — one or two caregivers live at the home and get a reasonable night's sleep (a private place to rest and uninterrupted hours), but are available if your loved one needs them in the night.

Which one fits your situation?

Choose 24-hour care if your loved one:

  • Wakes frequently, wanders, or has significant dementia (sundowning, night confusion)
  • Needs repositioning, toileting, or medication overnight
  • Is a high fall risk getting up at night
  • Is recovering from surgery or a hospital stay

Live-in care can be a great fit if your loved one:

  • Generally sleeps through the night
  • Needs companionship and help during the day and reassurance someone is there
  • Wants the continuity of one familiar caregiver
  • Is looking for a more affordable around-the-clock option

Cost comparison

Because 24-hour care staffs awake caregivers in multiple shifts, it costs more — you're paying for every hour, by several people. Live-in care is typically billed at a daily rate and is more economical when nights are quiet, since the caregiver sleeps. If overnight needs are heavy, though, live-in isn't appropriate — it wouldn't be safe or fair to the caregiver. See Pittsburgh home care rates and how many hours you actually need.

A quick way to decide

Ask: “In a typical night, how many times does my parent need help?” If the honest answer is “rarely” — live-in care likely fits and saves money. If it's “several times” — 24-hour care is the safe choice. Still unsure? We'll tell you straight after a free assessment. Call 412-318-4237 or book a visit.

Frequently asked questions

What is the difference between 24-hour care and live-in care?
24-hour care uses caregivers in shifts who stay awake the entire time, including overnight. Live-in care is one or two caregivers who sleep at the home (with a real night's rest) and are available if needed. The deciding factor is how much help the person needs overnight.
Which costs more, 24-hour or live-in care?
24-hour care costs more because it staffs awake caregivers across multiple shifts — you pay for every hour. Live-in care is usually a daily rate and more economical when nights are mostly quiet, since the caregiver sleeps. Heavy overnight needs require 24-hour care.
Is live-in care safe for someone with dementia?
It depends on the stage. If the person sleeps through most nights, live-in care can work well and offers comforting continuity. If they wander, wake frequently, or get confused at night (sundowning), 24-hour care with awake caregivers is the safer choice.
How do I decide between them?
Estimate how often your loved one needs help during a typical night. Rarely? Live-in care likely fits and costs less. Several times a night? Choose 24-hour care. A free in-home assessment from Mary Angels gives you a straight recommendation — call 412-318-4237.
