What Is VA Aid & Attendance?
The VA Aid & Attendance benefit is a monthly pension supplement paid by the U.S. Department of Veterans Affairs (VA) to wartime veterans — or their surviving spouses — who require assistance with everyday activities. "Aid & attendance" means the veteran needs another person's help to perform basic personal care tasks, such as:
- Bathing, grooming, and dressing
- Moving safely from room to room or getting in and out of bed
- Preparing meals or eating
- Managing medications (reminders, not administration)
- Protecting themselves from the hazards of daily living
This benefit is separate from VA health care and disability compensation. It is added on top of the basic VA pension, raising the maximum monthly dollar amount a qualifying veteran or surviving spouse can receive. The funds are flexible — families in Pittsburgh can use them to pay a licensed home care agency like Mary Angels Home Care directly.
Who Qualifies in Allegheny County?
Eligibility has four main pillars. Meeting all four is required:
Military service: The veteran must have served at least 90 days of active duty, with at least one day during a qualifying wartime period (World War II, Korea, Vietnam, Gulf War, and others). An honorable or general discharge is required.
Medical need: A physician must certify that the veteran needs help with activities of daily living, is bedridden, is a patient in a nursing facility, or has significantly impaired eyesight. Cognitive decline and dementia commonly meet this standard.
Income: Net income — after unreimbursed medical expenses including home care costs — must fall below the VA's annual pension limit. Because home care costs count as a deductible medical expense, many families whose gross income seems too high will qualify once care costs are factored in.
Assets (net worth): As of 2024, the net worth limit is $155,356 (adjusted annually). A primary home, vehicle, and certain personal effects are generally excluded from this calculation.
Surviving spouses of qualifying veterans may also be eligible for a lower-rate benefit. If you are unsure whether your parent qualifies, a VA-accredited claims agent or the Allegheny County Veterans Services office can review the situation at no cost.
How the Benefit Pays for Home Care in Pittsburgh
Once approved, the VA sends a monthly check directly to the veteran or surviving spouse. There are no restrictions on which licensed home care provider you choose — the money is yours to direct. A few important points:
- Maximum rates (2024 approximate): A veteran who qualifies for full Aid & Attendance receives up to roughly $2,300/month; a surviving spouse receives up to roughly $1,480/month. Exact amounts are set annually by the VA.
- No co-pay to the VA for this benefit — it is a pension, not a loan.
- The benefit is not retroactive to the application date immediately — there is typically a processing wait, so applying early matters.
- Home care costs used to meet the income test can be documented during the application, which is why working with an agency that provides clear invoices is helpful.
Mary Angels Home Care serves families across Pittsburgh neighborhoods including the North Hills, Mt. Lebanon, Squirrel Hill, Monroeville, and many more areas of Allegheny County. Our team can provide the care documentation your family needs to support a VA claim.
How to Apply for Aid & Attendance
The application process is handled through the VA, not through a home care agency. Here is what the path typically looks like:
- Gather documents: DD-214 (discharge papers), marriage certificate if applying as a surviving spouse, physician's statement of care need, and financial records.
- File VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance) completed by the veteran's doctor.
- Submit VA Form 21-527EZ (Application for Pension Benefits) along with supporting evidence.
- Work with a VA-accredited claims agent or Veterans Service Organization (VSO) such as the Veterans Service Center in Oakland, PA, or the American Legion and VFW posts active throughout Allegheny County. These services are free.
- Wait for a decision — processing can take several months, so many families begin home care and document costs while the claim is pending.
Beware of any company that charges an upfront fee to file a VA pension claim — VA-accredited representatives are required to provide claims assistance at no charge.
Starting Home Care While You Wait for VA Approval
The VA approval process can be slow, and your parent's needs are happening right now. Many Pittsburgh families choose to begin in-home care immediately, keep careful records of all costs, and use those documented expenses to help meet the income threshold in their VA application.
If round-the-clock support is needed, our 24/7 & live-in care option can bridge the gap. For families where a spouse or adult child has been the sole caregiver, respite care can provide relief while paperwork moves forward.
Once VA Aid & Attendance payments begin, many families apply them toward ongoing personal care services — help with bathing, dressing, grooming, and safe mobility — provided by a Mary Angels caregiver in the veteran's own home.
